 Physician Release is required if you are under a physicians regular care for conditions such as Diabetes, Heart Condition (including pace makers), Liver Cirrhosis, Degenerative Disc Disease, Cystic Fibrosis, Cancer or High Risk Pregnancy (including "bed rest" of any kind) 

 PHYSICIAN CONSENT FORM IS REQUIRED BEFORE TREATMENT.
